If you have any questions about the study and your right as a research participant, you may ask me now or you may also contact Dr. Musa Sani Zakirai at Center for Research, Evaluation Resources and Development in Ile-ife, Nigeria.
At this time, do you want to ask me anything about the survey?
Thank you for agreeing to hear more about the survey.
This research is being conducted by Drs. Scott Radloff and Phil Anglewicz of the Johns Hopkins University, and Dr. Funmilola OlaOlorun on behalf of the Center for Research Evaluation and Resource Development, Ile-Ife. This study is sponsored by the Bill and Melinda Gates Foundation through the Bill and Melinda Gates Institute for Population and Reproductive Health, Johns Hopkins University, Baltimore, MD.
The purpose of this research is to learn more about your reproductive behavior, and how you and your household have been impacted by the coronavirus pandemic in order to provide government and other stakeholders with data to improve interventions and programs aimed at this population.
We are asking you to join our study because you were interviewed by a member of our team in December 2019/January 2020 for a similar survey, and agreed to be contacted again. If you agree to participate in the study, we will ask you some of the same questions we asked before, as well as how this virus has impacted you. The questionnaire should take about 30 minutes to complete. After you complete the survey, you will be provided with recharge cards worth N300.
In total, we expect you to be involved in the original research for which we first contacted you for 3 years. Your participation in the study will only include annual or biannual surveys in-person or by phone.
There are no major risks to participating in the study, but you may get tired or bored when completing the questionnaire. Some questions may be personal or sensitive and may make you feel embarrassed or uncomfortable. Please let us know if you feel distressed.
There is a small risk that someone outside the study will see your information. We will do our best to keep your information safe by not including your name anywhere in the survey. Upon completion of the survey, data will be sent to and stored on a secure, password-protected server. Only authorized individuals will be able to access the data. All data are encrypted when transferred. When we share your information with other researchers, we will ask them to use the same protections.
Your participation in this research will not cost you anything, apart from some of your time.
There is no direct benefit to you from being in this study. We hope this knowledge will help us understand about reproductive health issues among women in Lagos, and how the coronavirus is affecting women's lives. The data collected from this study will provide information for future research, interventions, and programs.
All information collected in this study will be given code numbers, and no names will be recorded. Your responses cannot be linked to you in any way, and your name or any identifier will not be used in any publications or reports from this study.
Your participation in this research is entirely voluntary. You do not have to join, it is your choice. There will be no penalty if you decide not to join.
If you choose not to participate, this will not affect your ability to participate in other surveys or receive services in your community.
You will be given a small token for your time. You will receive recharge cards worth ?300 for participating in the study.
You can also choose to withdraw from the research at any time. Please note that any information that has been obtained from you before you choose to withdraw may have been used in data analysis or report writing. These cannot be removed, but the researchers will comply with your wish as much as it is practicable.
If you suffer any injury as a result of your participation in this research, the project will ensure you are referred to appropriate services.
What happens to research participants and communities when the research is over? The researchers will post the outcome of the research on the project website. During the course of this research, you will be informed about anything that may affect your continued participation, or your health.
If this research leads to commercial products, the Johns Hopkins University shall own it. There is no plan to contact any participant now or in the future about such commercial benefits.
We are not aware of anything that may prevent the researchers from conducting the research in a manner that will be fair.
Statement of person giving verbal consent: The description of the research has been read to me, or translated into language I understand. I have also talked it over with the interviewer to my satisfaction. I understand that my participation is voluntary. I know enough about the purpose, methods, risks and benefits of the research study to judge that I want to take part in it. I understand that I may freely stop being part of the study at any time. I am willing to participate.
This research has been approved by the Lagos State University Health Research Ethics Committee (LASUTH HERC) and the committee can be contacted at Health Research and Ethics Committee, Lagos State University Teaching Hospital, Lagos State. The phone number of the committee administrator is. In addition, if you have any question about your participation in this research, you can contact the local principal investigator, Dr. OlaOlorun, at her office in the Department of Community Medicine, College of Medicine, University of Ibadan.
Thank you for agreeing to hear more about the survey.
There are a few things you should know about this survey:
1. You get to decide if you want to be in the survey and whatever you decide is OK.
2. It is also OK to say ?Yes' and change your mind later. You can stop being in the survey at any time. If you want to stop, please tell me and I will not be upset.
3. You can say ?Yes' to the study and as I ask you questions, you can say ?No' to any question that you do not want to answer.
4. Your answers will be kept strictly confidential. That means that your answers will just be between you and me and will never be connected to your name or any other personal information.
5. Sometimes you might not know the answer to a question or might not want to answer a question. I would rather you say you don't know or that you don't want to answer a question than tell me a story that isn't true.
6. We will document your address so we can visit you at least one time a year for the 4 year duration of this study.
I wanted to give you some additional information on the type of questions I will ask you in this survey.
I want to tell you that some of the questions I will ask you are sensitive. I will ask you about your family, marital status and educational attainment. In addition, I will be asking about your sexual activity, pregnancy, fertility preferences and contraception. I know that these are sensitive and sometimes difficult things to talk about, but your answers will help us learn more about women in Nigeria.
The interview will take about 45 minutes to answer the questions.
There is little or no possibility that bad things will happen as a result of answering these questions. As I told you, some of the questions are sensitive and answering questions like this can be difficult, but you can choose not to answer questions that are difficult or end your participation at any time.
There are no direct benefits for being in the survey. This means that you will not get any money or gifts for being in this survey.
The interviewer has told me about the survey research. I had a chance to ask questions. I know that being in the survey is my choice. No one will be upset if I don't want to be in the survey or if I change my mind later. I know I can ask questions any time. I know that my answers will be kept confidential. I want to be in the survey.
