Hello. My name is __________________________ and I am working for the Center for Research, Evaluation Resources, and Development. We are conducting a local survey that asks women about various reproductive health issues. We would very much appreciate your participation in this survey. This information will help us inform the government to better plan health services in this and other areas of Kano, Nigeria. This survey will take about one hour to complete. The questions will cover family planning, women empowerment and health service delivery. In addition, there are 20 questions about the types of information related to family planning you may be interested in getting on your own that will be used for public health research purposes. You do not have to respond to these questions as part of the PMA survey. You will be requested to participate in follow-up surveys. In order to identify you in the future, we would like to keep your name, geographic information, and phone number. However, we will not share this information with anyone outside of the study team and we will destroy it after the follow-up surveys.
Participants in this survey are selected from various areas throughout Kano, Nigeria. Participation in this survey is entirely voluntary. I know that these are sensitive and sometimes difficult things to talk about, but your answers will help us learn more about women in Nigeria.

The interview will take about 45 minutes to answer the questions.

There is little or no possibility that bad things will happen as a result of answering these questions. As I told you, some of the questions are sensitive and answering questions like this can be difficult, but you can choose not to answer questions that are difficult or end your participation at any time.

There are no direct benefits for being in the survey. This means that you will not get any money or gifts for being in this survey.

The purpose of this research is to learn more about your reproductive behavior, and how you and your household have been impacted by the coronavirus pandemic in order to provide government and other stakeholders with data to improve interventions and programs aimed at this population.

We are asking you to join our study because you were interviewed by a member of our team in December 2019/January 2020 for a similar survey, and agreed to be contacted again. If you agree to participate in the study, we will ask you some of the same questions we asked before, as well as how this virus has impacted you. The questionnaire should take about 30 minutes to complete. After you complete the survey, you will be provided with a small gift.

In total, we expect you to be involved in the original research for which we first contacted you for 3 years. Your participation in the study will only include annual or biannual surveys in-person or by phone.

There are no major risks to participating in the study, but you may get tired or bored when completing the questionnaire. Some questions may be personal or sensitive and may make you feel embarrassed or uncomfortable. Please let us know if you feel distressed.

There is a small risk that someone outside the study will see your information. We will do our best to keep your information safe by not including your name anywhere in the survey. Upon completion of the survey, data will be sent to and stored on a secure, password-protected server. Only authorized individuals will be able to access the data. All data are encrypted when transferred. When we share your information with other researchers, we will ask them to use the same protections.

Your participation in this research will not cost you anything, apart from some of your time.

There is no direct benefit to you from being in this study. We hope this knowledge will help us understand about reproductive health issues among women in Lagos, and how the coronavirus is affecting women?s lives. The data collected from this study will provide information for future research, interventions, and programs.

All information collected in this study will be given code numbers, and no names will be recorded. Your responses cannot be linked to you in any way, and your name or any identifier will not be used in any publications or reports from this study.

Your participation in this research is entirely voluntary. You do not have to join, it is your choice. There will be no penalty if you decide not to join.

If you choose not to participate, this will not affect your ability to participate in other surveys or receive services in your community.

You will be given a small token for your time. You will receive a small gift for participating in the study.

You can also choose to withdraw from the research at any time. If this research leads to commercial products, the Johns Hopkins University shall own it. There is no plan to contact any participant now or in the future about such commercial benefits.

We are not aware of anything that may prevent the researchers from conducting the research in a manner that will be fair.
